What if the merchant does NOT give any Entry Forms?

Customer must keep receipt of purchase (using BIBD Mastercard & ATM/Debit Card) from ANY MERCHANTS and ANY AMOUNTS, and collect the Entry Forms from BIBD Branches and deposit the Entry Form attached with receipt of purchase to the allocated deposit boxes .
